1. Completing the Form

- Ensure all addresses are completed in full and there are no gaps in the date history.

2. Agreement

- I understand that an Enhanced Access NI Disclosure costs £32. Should I be successful for the position for which I have applied I understand and agree that the company will initially pay this cost on my behalf and that this fee will be deducted from my first wage.
- I permit the company to complete my e-application on my behalf based on the information I have provided.
- I understand the significance of making a 'Barred List Check application' should I be on a barried list.
- I understand that a check will be made to confirm my identity and ensure i have the right to work in the UK.
- I give my permission to the company to progress with an application that involves a barred list check.
- I understand that as deemed appropriate by the company it may be neccessary for an Enhanced Access NI re-check to be carried out. I give my permission in such circumstances for relevant Enhanced Access NI fee to be deducted from my wages.

Failure to complete this pack correctly may render your job application invalid.

Should you be successful in obtaining a position wiht the company any offer will be subject to receipt of a satisfactory Enhanced Access NI and employment may not commence until the disclosure has been received. You will be required to complete an application for an Enhanced Access NI check if you are invited to interview.
